1. About MyTelePep

MyTelePep is a telehealth service operated by TeleEMC d/b/a First Visit MD (a service of Luxe Inventions LLC), located at 4800 N. Federal Hwy., Suite 105B, Boca Raton, FL 33431, licensed as a health care clinic under the Florida Agency for Health Care Administration (AHCA). We provide physician-supervised telehealth consultations and all-inclusive peptide therapy programs to Florida residents only.

MyTelePep is not an emergency medical service. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

2. Eligibility — Florida Residents Only

To use MyTelePep's services, you must:

• Be 18 years of age or older
• Be a resident of the State of Florida
• Be physically located in Florida at the time of your telehealth consultation
• Have a valid email address and the ability to participate in a HIPAA-compliant video visit

By enrolling in a program, you represent and warrant that you meet all eligibility requirements. Providing false information about your location or identity is a violation of these Terms and may result in immediate termination of services.

3. Nature of Services — Telehealth Only

All consultations and follow-up care are delivered exclusively via HIPAA-compliant video. Telehealth consultations do not include physical examinations.

4. All-Inclusive Program Pricing

Program Fees — What's Included

• Wellness Program: $199/month — physician evaluation, 1 peptide protocol, 30-day medication, shipping, 1 follow-up visit
• Performance Program: $349/month — physician evaluation, up to 2 protocols, 30-day medication, shipping, 2 follow-up visits, baseline labs
• Longevity Premium: $549/month — physician evaluation, up to 3 protocols, 30-day medication, shipping, unlimited follow-ups, comprehensive labs, priority access

Optional Add-Ons

Optional add-ons (Advanced Labs +$79/mo, Sexual Health +$99/mo, NAD+ +$129/mo, Aesthetics +$69/mo) are billed in addition to your base program fee.

Cancellation Policy

You may cancel your program at any time with no cancellation fee. Cancellations take effect at the end of your current billing cycle. No partial-month refunds are issued. If physician approval cannot be obtained after your initial evaluation, we will refund your first month's program fee in full.

5. No Guarantee of Specific Protocol

Enrolling in a program does not guarantee a specific prescription or peptide compound. Our providers exercise independent clinical judgment and will only prescribe medications they determine are clinically appropriate for you specifically.

6. Compounded Medications — Important Disclosures

• Compounded peptides are not FDA-approved drugs and have not been evaluated by the FDA for safety, efficacy, or therapeutic equivalence
• Compounded medications are prepared by licensed 503A compounding pharmacies by prescription only
• Results from compounded peptides vary and are not guaranteed
• You acknowledge you have been informed of the non-FDA-approved status of any compounded medication prescribed

7. Patient Responsibilities

• Provide accurate, complete, and truthful health history and information
• Disclose all current medications, supplements, and health conditions
• Be physically located in Florida at the time of your consultation
• Participate in consultations from a private location
• Notify your provider of any adverse reactions or concerns after treatment begins
• Maintain care with your primary care provider for ongoing routine health needs

8. Limitation of Liability

To the fullest extent permitted by Florida and applicable law, MyTelePep, its officers, providers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability for any claim shall not exceed the amount you paid for the specific billing period giving rise to the claim.

9. Governing Law

These Terms are governed by the laws of the State of Florida. Any disputes shall be resolved in the state or federal courts located in Palm Beach County, Florida.

10. Changes to These Terms

We may update these Terms at any time. The updated version will be posted with a revised effective date. Continued use of our services after changes are posted constitutes your acceptance of the updated Terms.
